Justipedia explains Incident Report

One place that incident reports frequently occur is in hospitals. Hospitals commonly generate incident reports if an unusual occurrence happens. For example, if a victim comes into the hospital with four gunshot wounds, then the hospital may create an incident report. This incident report can then be given to law enforcement to help with the investigation. An incident report may also be brought up by one of the parties in a criminal lawsuit, to be used as evidence.
